Established in 2023 and affiliated with the Sri Sathya Sai University for Human Excellence (SSSUHE), it is distinguished as the first private medical college in India to offer completely free-of-cost medical education to all enrolled students. The institute operates with the vision "In Rural – For Rural," aiming to produce values-based doctors who are dedicated to serving underserved communities, particularly in rural India. Its educational approach is holistic, combining modern medical curriculum guidelines from the National Medical Commission (NMC) with a strong emphasis on human values, ethics, and selfless service.
The graduates are required to sign a bond committing to a few years of service at one of the trust's free hospitals in rural or medically underserved areas after completing their course. SMSIMSR is associated with a teaching hospital, the Sri Sathya Sai Sarla Memorial Hospital, which provides world-class, multi-specialty healthcare entirely free of charge. This unique model addresses the twin challenges of expensive medical education and the critical shortage of doctors in rural regions.

When medical education becomes a commodity that is sold and bought, often students are conditioned to commercialise their knowledge and skills during their medical practice, resulting in urbanisation and commercialisation of Healthcare and medical brain drain. The Medical College shall provide value based medical education, inspiring youth to serve the society and contribute towards Universal Health, with love and compassion as the guiding tenets.
